The purpose of this form is two-fold: To help us better understand you business and your target market, while simultaneously helping you think about your business and your place in the marketplace differently– which is a good thing!

Do I need to fill out the entire form?

Only the fields marked with an "*" are required. Some of the information may not be available at the time of filling out the form. If this is the case, simple enter "N/A" in the provided field and we will acquire this information from you at a later date.

Will I receive a copy of my answers?

Though a coipy will not automactically be sent you, we will gladly provide you with a copy of the questions and answers; all you have to do is ask. 😉
